- At least two of the following documentary evidences which may show the name of the child, date and place of birth, and the names of the parents:
- Baptismal Certificate
- Voter’s Certification (COMELEC)
- Valid Identification Cards (with date and place of birth)
- School Record
- SSS records
- Barangay Captain Certification
- NSO Negative Certification of Birth
- Certificate of Marriage, if Married
- Affidavit of Late Registration
- Community Tax Certificate [ Cedula ]
